Diploma Reorders Via Parchment/Instructure
Farmingdale State College has partnered with Parchment to accept online diploma reorders. You may reorder your official diploma or certificate 24 hours a day, 7 days a week. Your issued credential may be sent electronically or by mail in hard copy. A $25.00 fee is required to reorder your diploma.
Students may choose to have their legal or chosen first name printed on their initial (first-time) diploma or certificate. Diplomas and certificates can only be printed with the chosen first name designated at the time of graduation. In order to have the chosen first name printed on the diploma or certificate, the chosen first name must be requested to the Registrar’s Office and recorded in the student’s official personal information record prior to requesting the diploma or certificate. Only chosen first names can be printed on the diploma or certificate, legal last names are used for all official records, including diplomas and certificates.
To choose or change first name, pronouns or gender designation students must complete the Chosen Name, Pronouns and Gender Designation Form and submit it to the Office of the Registrar.
It is important to consider that some professional and licensing boards require use of legal name in the licensing process, so use of a chosen name may delay or complicate a certification or licensure process. In addition, some countries may require a diploma for various legal, immigration, or employment purposes. Official transcripts will always display legal names. This may cause discrepancies if a third party verifies both your diploma and transcript.

How to Reorder Your Diploma
Option 1: Through OASIS
Reorder your diploma online through your OASIS account.
If you need to reset your password, please contact the Registrar’s Office at 934-420-2776.

Option 2: Through Parchment
Purchase a duplicate diploma through Parchment
If you provided a mobile phone number, you may receive an SMS notification when your
order is processed. All notifications are sent via Parchment to the contact information
you provide during the order process. Please include a valid email address.
Important Information
If your legal name or address has changed, you must also complete the Change of Student Information Form.
If you have a hold due to a financial obligation, your request will be placed on hold until the obligation is resolved. You may check your account status through OASIS.
If you require proof of graduation prior to receiving your diploma, you may request an official transcript.

Third-Party Verifications
Third parties requesting enrollment or degree verification may use the National Student Clearinghouse. The National Student Clearinghouse is the College’s approved and reliable source for verification.
