On-Campus COVID Testing

Dear Student,

The College is making COVID-19 testing available at no cost to you, every Monday beginning Monday, August 24 until further notice. We urge you to take advantage of this opportunity to protect yourself and others. Testing will be conducted in a tent behind the Health & Wellness Center (HWC) from 9:00 am - 2:00 pm. This campus service is being provided in partnership with Enzo Clinical Labs. Test results take approximately 48-72 hours and are you will be notified of results by phone from an HWC staff member. Confidentiality is maintained. No appointment is necessary, but the following documents are required:

  • Farmingdale State College ID
  • Insurance Card

Upon arrival, you are required to wear a face-covering at all times and maintain a six-foot distance from others in the line. For any questions, please call the Health & Wellness Center at 934-420-2009.

If you would like to be tested at any other time, you may do so by appointment with Enzo Lab. Please call the Health & Wellness Center and a prescription will be sent digitally to Enzo Clinical Labs. An informational sheet will be emailed to you and you can then call the lab to make an appointment to be tested Tuesdays thru Thursdays. When making the appointment state that you are an FSC student and you will receive an expedited appointment. The HWC will relay the results of the test to you by phone.

Coronavirus Update: Health & Wellness Center

Latest Information on Coronavirus

Health and Wellness will begin seeing students for a limited number of appointments beginning the week of 7/20/20. No walk-in appointments will be allowed, you must call in advance for appointments or visit the Health and Wellness Center for any reason.

We will be screening all visitors for symptoms of COVID-19 and requiring all staff and patients to wear masks. Currently, no one is allowed to accompany patients to their appointments. These policies are in place to minimize the risk of COVID-19 spread to you, your family, other patients, and our staff.

When scheduling an in-person appointment, you'll be required to fill out the student self-screening tool prior to coming to campus (or leaving your residence hall room). The screening tool can be found on a banner after signing in to myFSC or here. We will ask the same questions when you arrive for your appointment, and we will also take the temperature of every patient and visitor who arrives at Health and Wellness using "no-touch" thermometers.

Upon arriving at Health and Wellness for your appointment, please call and let us know that you are on campus for your appointment. We are limiting the number of people in our waiting room, so you will be asked to wait outside or in your car until the staff at Health and Wellness is ready to see you.

Our doctor and nurse practitioner will also offer video visits. If you have questions, Health and Wellness can help you determine whether the appointment you need can be a video visit rather than in person.

Health and Wellness will be offering screening for COVID-19 and antibody testing for both employees and students through Enzo Labs to help keep our campus community safe.


The Health and Wellness Center is here for YOU! We are here to provide Farmingdale State College students with a wide variety of health services. A student with a current Farmingdale ID and the necessary immunization and health information are eligible to obtain these services.

  • Entrance physical exams
  • Women's health services (Gynecological exams)
  • Immunizations- MMR, tetanus, meningitis, influenza
  • Athletic physical exams and EKGs

Farmingdale State College is no longer offering a school-sponsored insurance plan for the 2020-2021 Academic year.

Assistance for Students With Hunger Issues

If you are having a hard time affording food, please contact Jamie Hawkins. Ms. Hawkins may be able to assist you in obtaining services in the community. You may also be eligible for SNAP (Supplemental Nutrition Assistance Program) You can apply online at Supplemental Nutritional Assistance Program External link (SNAP). Health and Welfare Counsel of Long Island is available to help you complete the application.    

Assistance for Student Wanting to Quit Tobacco Use

Important Registration Requirement

In order to quickly meet with your academic advisor and to register for classes, you must first provide the Health and Wellness Center with proof of receiving 2 MMR (measles, mumps, and rubella) immunizations. You must also provide us with proof of a meningitis immunization within the last five (5) years, or sign a meningitis response form (Meningitis Response and Information Form). Please be advised that resident students must have the meningitis immunization. 

You can obtain proof of these immunizations in several ways:

  • Your pediatrician or healthcare provider
  • A former high school or college
  • Veterans can get proof through the VA

If for some reason you cannot get a copy, contact your healthcare provider or make an appointment at the Health and Wellness Center. Blood work, called titers, will be drawn in order to determine immunity.

We accept your immunizations on our own immunization form (Immunization Form), your healthcare provider's form or on a school's form. Please make sure all documents are legible, signed and stamped by the healthcare provider, include your name and date of birth and RAM number (if known).  

Resident students will not receive keys to their dorm and athletes will not be allowed to practice until a complete Health Record is submitted.
