Any fees not covered by your insurance are the student's responsibility.*

Farmingdale State College no longer offers a school-sponsored health insurance plan. Students seeking health coverage are encouraged to explore options through the New York State of Health marketplace.

Important Health Requirement for Registration

Before you can meet with your academic advisor and register for classes, you must submit the following immunization documentation to the Health and Wellness Center:

  • Proof of two (2) MMR (Measles, Mumps, and Rubella) vaccinations
  • Proof of a meningitis vaccination within the past five (5) years, or a completed Meningitis Response and Information Form

Note: All resident students are required to have the meningitis immunization.

Where to Obtain Your Immunization Records:

  • Your current or former pediatrician or healthcare provider
  • Your previous high school or college health office
  • Veterans may request records through the Department of Veterans Affairs (VA)

If you're unable to obtain your records, contact your healthcare provider or schedule an appointment with the Health and Wellness Center. A blood test (titer) can be performed to confirm your immunity.

All documentation must be:

  • Clearly legible
  • Signed or stamped by a licensed healthcare provider
  • Include your full name, date of birth, and RAM number (if known)
