What do I need to be in compliance with my health records?

I am a part-time student (up to 11 credits).

1. Proof of 2 measles, mumps and rubella (MMR) immunization. First shot after your 1st birthday.

2. Meningitis immunization within 5 years or a Meningitis Response Form, which can be found in Important Forms. If you do not wish to have the meningitis immunization, you can waive it on the Meningitis Response Form.

I am a full-time commuter student (12 or more credits.)

1. Proof of 2 measles, mumps and rubella (MMR) immunization. First shot after your 1st birthday.

2. Meningitis immunization within 5 years or a Meningitis Response Form, which can be found in Important Forms. If you do not wish to have the meningitis immunization, you can waive it on the Meningitis Response Form.

3. Recommended- complete health record found in Important Forms. A complete health record includes immunization, laboratory testing and a physical.

I am a resident student.

1. Proof of 2 measles, mumps and rubella (MMR) immunization. First shot after your 1st birthday.

2. Meningitis immunization within 5 years. Resident student are required to have the meningitis immunization. The meningitis immunization can not be waived for resident students.

3. Required- complete health record found in Important Forms. A complete health record includes immunization, PPD and a physical.

I am an athlete playing on a FSC team.

1. Required- Complete health record found in Important Forms. A complete health record includes immunization, PPD, a physical and a Meningitis Response Form. Physical must be valid for entire school year in which you are participating, not just for you season.

2. Complete a Cardiac Athletic Questionnaire. Any "yes" responses require clearance from a Cardiologist. The form can be found under Important Forms on our website.

3. Please note: an updated athletic physical is required every year. The form for returning Athletes can be found in Important Forms on out website.

4. Clearance cards are obtained from Health and Wellness once the physical and Cardiac Athletic Questionnaire are completed. Sickle Cell Waiver forms are signed in Athletics.

I am a nursing, dental hygiene or medical laboratory technology student.

1. complete health record. A complete record is required every year.

2. Titers MUST be submitted, with lab report, for the following: Measles, Mumps, Rubella, and Varicella, Hepatitis B (or proof of shots.)

3. All forms are to be submitted through Castle Branch system. Note: 1st year students Nursing and Medical Lab Tech students must also submit copies to the Health and Wellness Center. Dental Health students must submit records to Health and Wellness each year.

What is an MMR?

MMR stands for Measles, Mumps and Rubella. It is an immunization most people receive in childhood. It is a series of 2 shot. The first shot must be given after your 1st birthday.

When is my immunization requirement due?

You must submit your MMR immunization or proof of the Meningitis vaccine (within 5 years), or a Meningitis Response Form before you register. Resident students must submit proof of Meningitis immunization within 5 years, a waiver will not be accepted.

Can I register for classes before I submit my immunization requirement?

No. You must submit you immunization requirement before you are able to register for classes.

Where can I get a copy of my immunizations?

You can get these records from your pediatrician, high school or from a previous college you attended. If you are unable to get these records, you can have blood work drawn for MMR titers and submit the lab report showing immunity.

How can I submit my records?

  1. Due to the emerging situation with the coronavirus and limited personal on campus the most effective way to submit immunization records is through email. Submitting to wellness@farmingdale.edu will allow the Health and Wellness staff to process your records in a timely manner.
  2. Mail your records to: Farmingdale State College, Health and Wellness Center, 2350 Broadhollow Rd, Farmingdale, NY 11735
  3. Fax your records to: 1-934-420-2137
  4. Email your records to: wellness@farmingdale.edu
  5. You may bring them directly to the Health and Wellness Center. If the office is closed there are secure mailboxes at the front and rear of the building
Please make sure your name and RAM number appear on the paperwork.

How can I verify that you received my records?

If you faxed your records, please allow 48 hours for the records to be processed, then call the office at 934-420-2009. If you mailed your records, please allow 7 business days before calling 934-420-2009 for verification.

Can I get my immunizations and Health Record done at the Health and Wellness Center?

Yes, you may have your immunizations and health record completed at the Health and Wellness Center. There is a fee for the service and an appointment is required. Please call 934-420-2009 for an appointment.

If I feel sick, can I just go over the Health and Wellness Center?

We prefer you make an appointment, but if you feel sick and it is urgent, you can always come in during the day and be seen by one of the nurses. Please call for an appointment for the doctor or nurse practitioner since they are not always in the office.
