Distance Learning Incentives for Online Course Development Spring 2022

Schedule for Spring 2022 Incentive Programs

· Approved UP-8 forms due back to the Distance Learning Office with all signatures from chairperson, HR, Budget & Finance and Provost’s Office · As per HR and SUNY guidelines, completed and signed UP-8 forms are required as part of this incentive prior to work beginning |
February 21, 2022 |
· Orientation with Distance Learning (11 AM – 12 PM) |
February 28, 2022 |
· Module 1 due (for new faculty) |
March 1, 2022 |
· The original, notarized Work for Hire agreement with wet signature must be dropped off or mailed to the Office of Distance Learning |
March 7, 2022 |
· Module 2 due (for new faculty) |

Program Information & Eligibility
1. |
Application proposal submission in Axiom: first come only and will close once limit has been reached |
2. |
Application will be reviewed and must be approved by department chair and dean |
3. |
Applicant eligibility will be reviewed by Human Resources and Administration Finance offices |
4. |
Faculty developer will submit an original, notarized Work for Hire agreement for co-ownership of the course development that is done prior to payment being issued as per SUNY/HR guidelines. Work for Hire agreement is attached for review |
5. |
Faculty developers will meet with an instructional designer via Collaborate throughout the process and adhere to all development deadlines |
6. |
Priority will be given to high-need courses for online (general education and core courses in the program) |
7. |
Faculty developers will meet with an instructional designer via web-conferencing throughout the process and adhere to all development deadlines |
8. |
Faculty must complete the FSC Orientation & Training for teaching online/hybrid if they have not done so already |
9. |
Faculty cannot develop the same course unless they are working together and sharing the stipend |
10 |
Courses on the attached list are not eligible for the incentive |
11 |
Faculty will develop and curate original content exclusive from publisher/third- party content in every learning module of the course development shell as part of the course development incentive program (examples include: original PowerPoint presentations, lectures in audio/video/text) |
Instructions for Applying for Incentive Program via Axiom Mentor:
- Proposals are submitted through Axiom Mentor:
- Go to Axiom Mentor at farmingdale.edu/axiommentor
- Course Syllabus must be uploaded with Application
- Incentive Online Course Development Application must be approved by Chair & Dean
- Select the Applications Tab
- On the Submission screen you will see the heading "Grant/Incentive/Award Applications" and beneath it, links to "Distance Learning Incentive for Online Course Development Spring 2022"
- Click the link and select the tab at the top of the page "Start New Application"
The deadlines and requirements are from SUNY and HR along with
development deadlines from Distance Learning. I f these requirements are not met for any reason, it will result in the cancellation of the
incentive.
