Please ask two professors to send a letter of recommendation to studyabroad@farmingdale.edu (academic advisors, coaches, or other college staff are acceptable too)
*If you are a Farmingdale student planning on studying abroad through another SUNY school, you will need to apply to that SUNY school first. When you are accepted into the program, complete the forms in Brightspace.
FSC STUDENTS, you will be enrolled in a Brightspace unit where you'll complete and upload a series of forms. Other SUNY students will complete the forms via email.

| Judicial Conduct Review Form |
All students must be in good academic and disciplinary standing to study abroad. Download the form, fill in the information and email it to the Dean of Students (or equivalent administrator) at your campus. FSC students should email the completed form to Email for verification and signature. |

Request copy/copies of your unofficial transcript(s) from current or previous colleges. |
| Student Consent Form - GDPR | Read, fill in information, and sign. |
| Passport | Upload a clear scan of informational/picture passport pages. The expiration date of the passport must be at least 6 months after the time when you will return to the USA. For example, if your return trip to the USA is May 15, 2027 the expiration date must be at least Nov. 15, 2026. |
| Agreement & Release | Read, fill in information, and sign. |
| Student Health Information | This is a confidential form that details your health, medications, allergies etc. You may need to make an appointment with your health care provider for their signature. Fill in and sign. |
| Financial Agreement Form |
It is important that you understand all the costs associated with your study abroad program. You are responsible for paying the study abroad fees out of pocket or with the assistance of financial aid. For students who receive financial aid: Ensure your FAFSA is completed if you plan on using financial aid. Your financial aid package and eligibility for federal, state and institutional aid may be impacted and/or revised if you study abroad. If you enroll in courses abroad that do not count toward your degree, your financial aid will be impacted. Set up an appointment by emailing faoffice@farmingdale.edu and bring the approved course pre-approval form and program budget sheet. |
| Course Pre-Approval Form |
Meet with your academic advisor and the Study Abroad staff to assist you in choosing classes. For FSC Faculty Led courses, use the bottom chart. For all other programs, use the upper chart. List the course number/name in the first column and the number of credits in the second column. Ask the Department Chair who oversees similar courses at FSC to sign near the appropriate course (for example MATH 103 has to be approved by FSC's Chair of the Math department). That Department Chair will determine a Farmingdale equivalent course and confirm the number of credits. You may be asked to provide a syllabus. Once you obtain signatures for the classes, the Chair of the department of your Major will determine if the course(s) will apply to your degree program. Since the overseas host institution may cancel or alter the course(s) at any time, it is important to be flexible and receive approvals for alternate courses (so choose more than 4-5 courses, if possible). You will need this form to request financial aid. |
