- Select the semester you will be off campus for Study Abroad
- Click Submit Application. This will open the application page.
*If you are a FSC student planning on studying abroad through another SUNY school, you will need to apply to that SUNY school first. When you are accepted into the program, complete the FSC application in Axiom.

Judicial Conduct Review Form |
All students must be in good academic and disciplinary standing to study abroad. Download the form, fill in the information and email it to the Dean of Students (or equivalent administrator) at your campus. FSC students should email the completed form to Email and then upload the signed copy to Axiom. Other SUNY students, email the signed form to us at studyabroad@farmingdale.edu |
Letters of Recommendation |
Two letters of recommendation from professors who taught you at the collegiate level are required. Please contact your professors and request that they send a letter of recommendation to studyabroad@farmingdale.edu |
Unofficial transcript(s) |
Request copy/copies of your unofficial transcript(s) from current or previous colleges. FSC students need to submit a request to the Registrar's office and then upload the PDF to Axiom. Other SUNY students will email the PDF to studyabroad@farmingdale.edu |

Course Approval |
THIS IS ONLY FOR FSC STUDENTS. SUNY Students of other campuses will submit a similar form to their home campus. The courses you take at the overseas institute are not automatically transferred to your Farmingdale degree program. In order to make sure the courses you take overseas are applied to your degree, the overseas courses must be evaluated to determine if the course will transfer to Farmingdale. This is done using the course approval form. You will need to bring each course description/syllabus to the Department Chair that oversees similar course subjects at FSC (for example MATH 103 has to be approved by FSC's Chair of the Math department). That Department Chair will determine a Farmingdale equivalent course. The Chair of the department of your Major will determine if the course(s) will apply to your degree program. Since the overseas host institution may cancel or alter the course(s) at any time, it is important to be flexible and receive approvals for alternate courses (so choose 8-9 potential courses, if possible). Credit is not earned for repeated courses. Repeat courses may impact financial aid eligibility. You must receive a grade of āCā or higher. The credits received abroad from another institution will be reflected on your Farmingdale State College transcript as transfer credits. These grades will not affect your GPA. If pre-approval is not received, degree completion, financial aid and fees from student accounts may be impacted. It is your responsibility to meet with their department chair. |

Passport | Upload a clear scan of both passport pages. The expiration date of the passport must be at least 6 months after the time when you will return to the USA. For example, if your return trip to the USA is May 15, 2025, the expiration date must be at least Nov. 15, 2025. |

STEP Confirmation | The Smart Traveler Enrollment Program (STEP) is a free service to allow U.S. citizens and nationals traveling and living abroad to enroll their trip with the nearest U.S. Embassy or Consulate. The STEP Program will provide you with important information from the Embassy about safety conditions in your destination country. Create an account online. Upload a screenshot of your account confirmation page. |
Acceptance Letter | FOR FSC Students: the host SUNY school will supply you with an acceptance letter. Upload a copy of the letter. |
Student Health Information | This is a confidential form that details your health, medications, allergies etc. You may need to make an appointment with your health care provider for their signature. Upload a copy of the signed form. |
Financial Agreement Form |
All students must submit a Financial Agreement Form. It is important that you understand all the costs associated with your study abroad program. All FSC study abroad programs grant academic credit, therefore the student will be billed according to the corresponding program budget sheet. Please note that your financial aid package and eligibility for federal, state and institutional aid may be impacted and/or revised if you study abroad. If you enroll in courses abroad that do not count toward your degree, your financial aid will be impacted. Visit the FSC Financial Aid Office early on in the study abroad process and ensure your FAFSA is completed if you plan on using financial aid. Set up an appointment by emailing faoffice@farmingdale.edu and bring the approved course pre-approval form and program budget sheet. Upload a copy of the signed form. |
