How to Navigate Accommodate

1) Click on link to form Link to Getting to Know You Form (Step One).

2) Complete all questions.

3) Upload your documentation by clicking “Add Item”.

4) Click Submit once complete.

1) Click on the link to Accommodate: Link to DSC Accommodations Hub.

2) Select Student Button on screen.

3) Enter your user name and password.
Your User Name is your Farmingdale email address and your Password is the same password you use to log into MYFSC.

4) Click Sign In.

1) Log into Accommodate.

2) Click on Upload Supporting Documentation on the Right Hand Side under Shortcuts.

3) Click on Add New Accessibility Document.

4) In Document Title type the Name of the Document. IE. If the document is an IEP type IEP, if its an Evaluation type the name of the evaluation.

5) Upload Document by either clicking on upload file and following directions or drag and drop the file from your computer to the box in the DSC Hub.

6) Click Save if you need to upload an additional document you have uploaded all the documentation required in one file click submit.

1) Log into Accommodate Link to DSC Accommodations Hub.

2) Click “Accommodations” on Left Side.

3) Select “Semester Accommodation Letter Request”.
If you do not see this option, please contact the DSC office.

4) Select “Add New”.

5) Verify the semester you are requesting accommodations.
You will see what accommodations you are approved for at the top in blue and the courses you are enrolled in on the right.  Click on “Review The Renewal”.

6) Remove any accommodations you don't need. 
For example if you don’t need the use of a calculator for English 101, under the Accommodation Use of Calculator you would uncheck English 101 and any other course you are taking that does not require the use of calculator.

7) Once you have completed your review, Click "Submit" at bottom of page

Your request is sent to DSC for review and to send letter to Professors.
You will receive a copy of that email as well.

What Happens Next?

  1. You will receive a confirmation email after you submit your request.
  2. Your request is sent to DSC for review.
  3. DSC will send accommodation letters to your Professors and you will receive a copy as well.
