Freshman Document Submission

Documents required for application:

  • High School Transcript
  • SAT and/or ACT Scores

Please talk to your guidance counselor about submitting your documentation digitally. Counselors can submit your documents digitally through the following:

  • SUNY Application
  • Naviance
  • Parchment Docufide
  • Other Document Submitting Software

 

Other Recommended Documents:

  • Letter(s) of Recommendation
  • Mid-Year Transcript
  • Final Transcript
  • Resume
  • Supplemental Application
  • College Credits (if credits are not from Farmingdale, please contact the college's registrar office to have that transcript sent)
  • AP Score Report sent through CollegeBoard
  • Misc. Documents

If you are home schooled, you will need to provide the following documents:

  • High School Transcript
  • SAT and/or ACT Scores
  • Letter from Home District
  • Home Schooled Applicant Letter from the District Superintendent

 

If unable to submit transcript digitally, you can send by mail to our scanning facility in Albany:
