International Student Health Insurance

All international students on a F1 or J1 visa are automatically enrolled in mandatory SUNY approved health insurance through United Healthcare (UHC). 

Students must have health insurance for the entire academic year without any gaps in coverage. Students must keep their health insurance active throughout the summer, even if they are not taking courses or leave the U.S. temporarily.

Automatic Enrollment and Billing

All international students are billed and enrolled into the SUNY approved health insurance. No action is needed for an international student to be enrolled into the SUNY approved health insurance.

Students on OPT have the option to enroll in SUNY approved health insurance. To qualify, you must have been enrolled in SUNY approved health insurance for at least the prior month at the time of enrollment. There can be no gaps in your coverage and no exceptions will be made for students who have gaps and, therefore, missed the chance to apply. Contact the International office for an enrollment form. 

Students with dependents are required to enroll their dependents in health insurance. You can only enroll your dependents after you have been enrolled in health insurance. Payment for your dependents is made to United Healthcare Student Resources directly. Contact the International office for an enrollment form.

Account Set Up

After you are enrolled, you will receive a welcome email from the insurance company with login and ID card access instructions.  The welcome email will come from United Healthcare Student Resources (UHCSR), notifications@uhcsr.com.

If you missed this email, you can go directly to www.uhcsr.com. Enter “Farmingdale State College” for your school and follow the instructions to complete your account setup.

Accessing Care

Your online account gives you 24/7 access to your health insurance information. Once you create your account, you can search for providers, review claim information, access member guides, etc. Please check your plan for information about co-pays, deductibles and other costs.

Take your insurance ID card with you to all medical appointments.
