What it is:                                                                                                                                    MS Forms is an application available through Microsoft 365, that allows you to quickly and easily create a variety of custom surveys, quizzes, registrations, and more.  Qualtrics is a research software program that can be used to create surveys with sophisticated analytics. Google forms is an application available through G-Suite for Education, that allows you to quickly and easily create a variety of forms.

  1. Go to Forms.Office.com
  2. Login with your FSC account
  3. Click on “New Form”

  1. Go to Farmingdale.Qualtrics.com
  2. Log in with your FSC account
  3. Click on “Create New Project”

Log in, in upper right corner or when prompted and selected “Sign-in with SSO”. Organization ID = Farmingdale, enter your FSC account information.

  1. Go to Forms.Google.com
  2. Log in with your FSC account
  3. Choose a new form
