April 15, 2021
Dear Student,
Farmingdale State College has been working closely with SUNY to secure doses of the
Pfizer vaccine for our current students. These are now available for our students age 16 or above at the state-run vaccine
site located at SUNY Old Westbury starting on Tuesday, April 20. The Pfizer vaccine requires two injections. Therefore, a student must confirm that
they are available for both vaccine appointments.
To secure your spot for the COVID-19 vaccine, you must first call the Farmingdale
State College hotline at 934-420-5500 between the hours of 10 a.m. and 3 p.m. beginning on Monday, April 19. This is reserved for current Farmingdale State College students. Please be prepared
with your Ram ID # when you call the hotline. Your identity will be verified over-the-phone.
Once registered, you will immediately receive your appointment for the first vaccine
to be administered at the SUNY Old Westbury site. Appointments are on a first-come first-serve basis. Once registered at the vaccine
site for the first appointment, students will be able to schedule their second appointment.
If all vaccine appointments are filled, a message will be posted on the COVID hub. We hope to secure more vaccines for our students and will update you when this occurs.
Your Vaccine Location:
SUNY Old Westbury Clark Athletic Center 223 Store Hill Rd. Old Westbury, NY 11568
We urge all students to schedule vaccine appointments by the end of the semester.
Please protect yourself, your family, and your community. We all want to see our campus
return to normal operations in fall 2021. Receiving vaccinations is the best way to
reach that goal.
To check for appointments at other sites that may be closer to you, visit am-i-eligible.covid19vaccine.health.ny.gov/. If you are initially unable to schedule an appointment continue checking as new
appointment slots are added often.
You can learn more about the vaccines and New York’s vaccination efforts at covid19vaccine.health.ny.gov or call the Farmingdale hotline 934-420-5500 to speak with a staff member.
