Dear Student,
New York State is working to distribute vaccines to all eligible New Yorkers, with
many SUNY campuses serving as vaccine distribution sites.
Farmingdale State College has been working closely with SUNY to secure doses of the
Johnson & Johnson vaccine for our students. These are now available for our students age 18 or above at the state-run vaccine
site located at SUNY Old Westbury from Monday, April 12 to Friday, April 16. The Johnson & Johnson vaccine requires only one injection; a second dose is not needed.
To secure your spot for the COVID-19 vaccine, you must first call the Farmingdale State College hotline at 934-420-5500 between the hours of 9 a.m. and 5 p.m. This is reserved for current Farmingdale State College students. Please be prepared
with your Ram ID # when you call the hotline. Your identity will be verified over-the-phone.
Once registered, you will immediately receive an appointment for the vaccine to be
administered at the SUNY Old Westbury site. Appointments are on a first-come first-serve basis. If all vaccine appointments
are filled, a message will be posted on the COVID hub. We hope to secure more vaccines for our students and will update you when this occurs.

We urge all students to schedule vaccine appointments by the end of the semester.
Please protect yourself, your family, and your community. We all want to see our campus
return to normal operations in fall 2021. Receiving vaccinations is the best way to
reach that goal.
To check for appointments at other sites that may be closer to you, visit https://am-i-eligible.covid19vaccine.health.ny.gov/. If you are initially unable to schedule an appointment continue checking as new
appointment slots are added often.
