2024-25 Faculty Annual Reports
Dear Colleagues:
It’s that time of year again to begin preparing your Faculty Annual Report. In accordance
with Article XII of the Policies of the Board of Trustees, all academic employees
must be evaluated. At Farmingdale, the Faculty Annual Report is the mechanism used for this evaluation. Information contained in this report is
considered when determining recognition for outstanding contributions to the College
and may be reflected in the College’s Annual Report. The Faculty Annual Report is
also the basis of faculty evaluation for reappointment, promotion, and discretionary
salary increases.
Annual Reporting is structured in Axiom Mentor using the category headings of Teaching, Scholarship, and Service. The annual report outline guide has been designed to coincide with these areas of responsibility and includes a list
of sample activities to consider. It is not intended that each activity must be included
in an individual’s annual report, but faculty can use this list of topics as a guide
for organizing their activities and accomplishments within each of the three broad
categories. Please use the item ‘other’ under Additional Reporting in Axiom Mentor to note meaningful activities that are not aligned with the listed
activity sections.
In concluding your report, please provide information on your progress in achieving
past reported short- and long-range goals and objectives. Information captured here
provides continuity in the progression and evolution of professional goals from year
to year. If a goal has not been achieved, reflect on the steps that you have completed
and outline the steps going forward. Lastly, update your goals, if needed, to reflect
any future professional goals and objectives in the areas of teaching, scholarship,
and service.
The deadline to complete your submission is June 6, 2025. Your chair will review the contents of the report, provide comments, and approve your submission
in Axiom Mentor by June 20, 2025. The dean will have until July 11, 2025, for final approval of their School’s faculty reports. Please contact your department
chair or school dean should you have any questions regarding this activity.
