Fall 2025 Student Feedback Surveys

To All Faculty:
As the semester starts to wind down over the coming weeks, please be reminded that
student feedback surveys are available for distribution in Axiom Mentor. The surveys
will close to students on December 12, 2025.
It is expected that faculty will have student feedback surveys for review processes,
such as reappointment, continuing appointment, and promotion. Additionally, faculty
members nominated for Chancellor’s Awards are required to submit student feedback
surveys as part of their nomination packet.
Students can complete the survey on a mobile device or computer, and class time can
be utilized for their completion. Using class time for surveys is a good way to increase
the number of students providing feedback.
If you have not used this function before, you can log in to Axiom Mentor through
your MyFSC portal. Instructions for completion can be found on the homepage of Axiom
Mentor: “How to Start your Student Feedback Survey.”
