Application Requirements for Study Abroad

Apply Online:  using Axiom Mentor

  • Log onto Axiom
  • Select Applied Learning
  • Select International Education and Programs Unit
  • Select the semester you will be off campus for Study Abroad
  • Click Submit Application. This will open the application page. 

If you plan on studying abroad through another SUNY school, you will need to apply directly to that SUNY school by submitting an application. You will still need to submit an application on Farmingdale’s portal too. 

To apply, upload the following documents to AXIOM.

Study Abroad Application This is a general application that is completed online.
Application Fee Form The application fee for all FSC-hosted programs is $60 and is non-refundable. Payment is made online using OASIS or take the Application fee form inperson to the Student Accounts office to process the payment. The student only pays one application fee to the administrating SUNY. Upload your payment confirmation to AXIOM.
Student Consent Form - GDPR Read and upload a signed copy. 
Judicial Conduct Review Form  All students must be in good academic and disciplinary standing to study abroad. Email this form to the Dean of Students for their signature. Upload a signed copy. 
2 Recommendation Letters Upload two letters of support. Recommendation letters from professors who taught the student at the collegiate level is strongly preferred. If the recommender prefers, they can use this recommendation form instead of writing a letter of support. 
Passport Upload a clear scan of both passport pages. The passport must be unexpired.
Agreement & Release Read and upload a signed copy.
STEP Confirmation The Smart Traveler Enrollment Program (STEP) is a free service to allow U.S. citizens and nationals traveling and living abroad to enroll their trip with the nearest U.S. Embassy or Consulate. The STEP Program will provide you with important information from the Embassy about safety conditions in your destination country. Create an account online. Upload a screenshot of your account confirmation page.
Acceptance Letter The host SUNY school may supply you with an acceptance letter. Upload a copy of the letter. 
Course Approval The courses you enroll in at the overseas institute are meant to apply to your Farmingdale degree program. Have the overseas courses evaluated to determine if the course will transfer to Farmingdale. This is done using the course approval form. The student will need to bring each individual course description/syllabi to the Department Chair that oversee similar course subjects at FSC. That Department Chair will determine a Farmingdale equivalent course. Your academic department chair will determine if the course will apply to your degree program. Your overseas host institution may cancel or alter the course(s) at any time. Therefore, it is important to be flexible and receive approvals for alternate courses. Credit is not earned for repeated courses. Repeat courses may impact financial aid eligibility. You must receive a grade of “C” or higher. The credits received abroad from another institution will be reflected on your Farmingdale State College transcript as transfer credits. These grades will not affect your GPA. If pre-approval is not received, degree completion, financial aid and fees from student accounts may be impacted. It is the responsibility of the student to meet with their department chair.  
Student Health Information This is a confidential form that details your health, medications, allergies etc. You may need to make an appointment with your health care provider for their signature. Upload a copy of the signed form.
Financial Agreement Form

All students must submit a Financial Agreement Form. It is important that you understand all the costs associated with your study abroad program. All FSC study abroad programs grant academic credit, therefore the student will be billed according to the corresponding program budget sheet. Please note that your financial aid package and eligibility for federal, state and institutional aid may be impacted and/or revised if you study abroad. If you enroll in courses abroad that do not count toward your degree, your financial aid will be impacted. Visit the FSC Financial Aid Office early on in the study abroad process and ensure your FAFSA is completed if you plan on using financial aid. Set up an appointment by emailing faoffice@farmingdale.edu and bring the approved course pre-approval form and program budget sheet. Upload a copy of the signed form.
