Information for New Incoming Student

Welcome to Farmingdale!

Here at the Disability Services Center we would like to help start your transition to Farmingdale State College.  

There are a couple of things you can do before we meet you in person:

1. Complete our "Getting to Know You" form.  Link to Form.
2. Submit your documentation about your disability.
   Scan and email documentation to our Office Manager, Email 
3. Schedule a time to meet with a staff member.
