The Human Resources Department assists faculty and staff in understanding their benefit programs.  We work closely with the Department of Civil Service, Division of Employee Benefits, SUNY System Administration, and the various carriers to ensure that the programs work smoothly and employees receive their maximum entitlement. 2018 Option Transfer Is Now Open

The 2018 NYSHIP Option Transfer Period runs from through December 15, 2017. During this period, employees enrolled in the New York State Health Insurance Program (NYSHIP) may change their plan option for the 2018 plan year. Option Transfer changes can be requested either online, or via a completed PS-404 form submitted to the Human Resources office.

The Opt-out Program allows eligible employees who have other employer-sponsored group health insurance, to opt out of their NYSHIP coverage in exchange for an annual incentive payment of $1,000 for waiving individual coverage, or $3,000 for waiving family coverage. For eligibility and additional program information, please see page 15 in the 2018 NYSHIP Choices booklet. To enroll, complete and submit the following forms to the Human Resources office by Friday, December 15, 2017:

Effective on November 3, 2017, Human Resources can no longer accept the Salary Reduction Agreement Form to process a new 403(b) enrollment or change an existing bi-weekly 403(b) contribution amount. Employees must register on the SUNY Retirement@Work website to process your request
