Online Registration:
There are two forms of Online Registration available:

* - Current students can registrar through our Online Student Information System. Note: For this method a Student Pin is required to access your records. If you do not have a pin, please call
631-420-2776 during business hours (Mon-Fri 9am-5pm). * - Visiting students (students matriculated at other schools) and students not in a program of study, can use our Email Registration Form . This is a fill-out form that is electronically submitted to the Registrars Office.

Transcript Information

All requests must be made in writing. Requests will not be processed if you have any financial obligations to the college.

All requests MUST be signed or transcripts WILL NOT be released.

NOTE: We do not hold transcript requests for any reason. If you are waiting for grades, grade changes or your degree, do not request the transcript until you are sure these are posted to your academic record.

Official Transcripts: A signature is required if you are mailing or faxing your request. The first 5 transcripts per request are free of charge; over 5 in that same request will be charged a $5.00 each. Same day pick-up for Official copies is available from 9am to 4pm, Monday through Friday. These requests MUST be made in person and there will be a waiting period. We cannot release transcripts WITHOUT a picture ID (driver's license or student ID)

Unofficial Transcripts: Student copies of transcripts are free of charge. A picture ID (driver's license or student ID) is required when picking up a student copy and a signature is required if you are mailing in your request.

Student Rights

Your Student Rights (FERPA)
Family Education Rights and Privacy Act of 1974

This act was designed to protect the privacy of students’ educational records. The Family Rights and Privacy Act (FERPA) affords students certain rights.

The Rights that are protected under this Act are as follows:

1) The right to inspect and review the student’s education records within 45 days of the day the College receives a request for access. Students should submit to the Registrar, Dean, head of academic department, or other appropriate official, written requests that identify the records they wish to inspect. The College official will make arrangements for access and notify the student of the time and place where the records may be inspected. If the records are not maintained by the College official to whom the request was submitted, that official should advise the students of the correct official to whom the request should be addressed.

2) The Right to request an amendment of the student’s education records that the student believes are inaccurate or misleading. They should submit a written request to the College official responsible for the record and clearly identify the part of the record they believe should be changed and specify why they believe the record is inaccurate or misleading. If the College decides not to amend the record as requested by the student, the College will notify the student of the decision and advise the student of his or her right to a hearing regarding the request for amendment. Additional information regarding the hearing procedures will be provided to the student when notified of the right to a hearing.

3) The right to consent to disclosures of personally identifiable information contained in the students education records, except to the extent that FERPA authorizes disclosure without consent. One exception which permits disclosure without consent is disclosure to school officials with legitimate educational interests. A school official is a person employed by the College in an administrative, supervisory, academic or research, or support staff position( including law enforcement unit personnel and health staff); a person or company with whom the College has contracted( such as an attorney, auditor or collection agent); a person serving on the Board of Trustees; or a student serving on an official committee, such as a disciplinary or grievance committee, or assisting another school official in performing his or her tasks. A school official has a legitimate educational interest if the official needs to review an education record in order to fulfill his or her professional responsibility.

The following under FERPA is considered “Directory Information” at Farmingdale State and will be made available to the general public unless the student notifies the Office of the Registrar in person or in writing BEFORE the Last Day to add classes.

4) The Right to file a complaint with the U.S. department of Education concerning alleged failures by Farmingdale State to comply with the requirements of FERPA.
