What do I need to be in compliance with my health records?

1. Proof of 2 measles, mumps and rubella (MMR) immunization.  First shot after your 1st birthday.

 4. Clearance cards are obtained from Health and Wellness once the physical and Cardiac Athletic Questionnaire are completed.  Sickle Cell Waiver forms are signed in Athletics.

MMR stands for Measles, Mumps and Rubella.  It is an immunization most people receive in childhood.  It is a series of 2 shot.  The first shot must be given after your 1st birthday.

You must submit your MMR immunization or proof of the Meningitis vaccine (within 5 years), or a Meningitis Response Form before you register.  Resident students must submit proof of Meningitis immunization within 5 years, a waiver will not be accepted.

You can get these records from your pediatrician, high school or from a previous college you attended.  If you are unable to get these records, you can have blood work drawn for MMR titers and submit the lab report showing immunity.

  4. You may bring them directly to the Health and Wellness Center.  If the office is closed there are secure mailboxes at the front and rear of the building

If you faxed your records, please allow 48 hours for the records to be processed, then call the office at (631) 420-2009.  If you mailed your records, please allow 7 business days before calling (631) 420-2009 for verification.

Yes, you may have your immunizations and health record completed at the Health and Wellness Center.  There is a fee for the service and an appointment is required.  Please call (631) 420-2009 for an appointment.

We prefer you make an appointment, but if you feel sick and it is urgent, you can always come in during the day and be seen by one of the nurses.  Please call for an appointment for the doctor or nurse practitioner since they are not always in the office.
