Curriculum Change - This form can be used to change majors AFTER you have already started attending Farmingdale State College.
Dual Degree - A student may pursue a second degree of the same level either concurrently or consecutively. A student seeking to earn two degrees must meet the admission requirements of both degree areas as listed in the College catalog and must receive advisement from the academic department responsible for the second discipline. At a minimum, an additional 30 credit hours must be completed in the second degree area which are significantly different from the first degree.
Dual Majors - A student wishing to major in two disciplines at the same degree level concurrently would be awarded a single degree based on the primary major. The student must meet all the admission requirements for both majors as listed in the College catalog and must receive advisement in the second area from the academic department responsible for the second discipline. Both majors will be listed on the official transcript and diploma.
Minor - Please seek approval from the department of the minor, as well as the department of your major.
Cancellation of Dual Degree/Major or Minor - Please file this form if you do not intend to complete the requirements for your minor or dual degree/major.
Off-Campus Approval - Are you interested in taking courses off campus? Have the courses approved by the department of your major before registering.
Application for Readmission - Use this form is you have left Farmingdale State College for less than five years and would like to return under a new major.
New Student Curriculum Change - This form can be used if you have not started attending classes. i.e. If you were accepted for Business, but decide you would prefer to start in Liberal Arts. Please fax completed form to Admissions at 631-420-2633
New Student Semester Change - You can use this form to defer an application for a future semester. An application can be deferred up to one year. Please fax completed form to Admissions at 631-420-2633.
Joint Admissions/Intent to Enroll - This form can be used if you are graduating with your associate degree and plan on continuing on to a bachelor degree program.
Official Transcript Request - Please fill out this form and return to the Reigstrar's Office. They are located in Laffin Hall or it can be faxed to 631-420-2275. For more information about Official Records please refer to the Registrar's Office information.
