International Admissions Directions
The following checklist will help you organize your application and the supporting documentation required for timely con-sideration of your application. Please read all the instructions carefully. A complete application consists of the following:
State University of New York application for Undergraduate Admissions.
- An Application fee of $50.00 must be paid by a credit card to SUNY ASC (SUNY Applications Services Center).
Complete the forms listed below and return them to Jeanne Soto:
Official secondary school and post-secondary records must be submitted in original language along with certified transla- tion. Farmingdale requires that documents are translated and evaluated by a member of NACES (
http://www.naces.org/). WES (
) is a preferred evaluation agency.
Send an official score report for the TOEFL (Test of English as a Foreign Language) directly to the Admissions Office. TOEFL must be taken by all students whose native languages are not English unless they have completed a college- level writing course at a U.S. college.
A carefully completed Declaration and Certification of Finances form FSA 4 (
Currently $30,000 is required for each year of study. Please note that Farmingdale State College does not offer financial aid or scholarships to international students.
If funds are being provided by family and/or sponsor(s), each sponsor must complete an Affidavit of Support form in its entirety and submit it along with financial documents
– This form must be notarized.
Submit a separate original bank letter or statement for each source of funds. Account statements and letters (current within the last 6 months) must show actual amount in account and be in U. S. dollar equivalency.
International Students Already Studying In The U.S.
F-1 students are required to submit:
- Photocopies of all FORM I-20s issued (pages 1 & 3 of SEVIS I-20s, pages 3 & 4 of Pre-SEVIS I-20s)
- Photocopy of both sides and current I-94
- Photocopy of the passport page that includes your identification photo (U. S. Visa page and home country’s page)
- International Students Transfer Report completed by your current school’s Designated School Official (DSO) or
International Student Advisor

Indicate on the SUNY application your curriculum choice exactly as listed to avoid a delay in processing, which could jeopardize your opportunity for admission. You may apply to only ONE program. The academic year at Farmingdale State College begins in August and ends in May. All programs accept new students for the fall semester.
Dental Hygiene and Nursing applicants must be U. S. Citizens or Permanent Residents in order to be considered for admission.
Decisions on applications are processed on a first-come, first-served basis; therefore, it is to your benefit to mail all materials immediately upon completion, since programs fill up quickly. To ensure adequate time for the processing of International Student applications, please note the Admissions Office must receive the completed applications by June 30 for FALL admissions and November 1 for SPRING admissions.
