Applying to PHPA

Please fill out all highlighted fields and you can save data typed into this form to your computer.  When done filling out form go to file and save as. Choose the destination and name file. If you encounter problems filling out form in your browser please download file to your desktop and then use Adobe Reader to open the file.

Matriculated Farmingdale students, who have a minimum GPA of 3.0 are welcome to seek advice and answers from the PHPA office.

Appointments can be made by calling Dr. Miles at (631) 420-2530 or email her at marie.hayden-miles@farmingdale.edu.

Students seeking application to Medical (M.D./D.O.), Dental, or Veterinary programs must formally apply to the PHPA program in order to be eligible for a composite evaluation file containing letters of recommendation. Other programs require individual letters of recommendation (Click here for details).

This must be done before a student starts to fill out the electronic common application of the respective program of study. Only students who are currently matriculated at Farmingdale State College are eligible to apply (alumni who applied while matriculated continue to be eligible for assistance).

The deadline for application to PHPA is April 1st. of the year a student intends to apply to health professions programs. The application will soon be available here and must be delivered with an official transcript to Ms. Deborah Faulhaber in Gleeson 247 upon completion.
